Microsoft Forms is a simple survey and quiz tool included in Microsoft 365. Access it at forms.microsoft.com. Create forms with multiple choice, text, rating, date, ranking, and Likert scale questions. No design skills required — Forms handles formatting automatically.
For IT use cases: create a hardware request form, new employee onboarding intake form, IT satisfaction survey, or incident report form. Share the form via a link, embed it in a SharePoint page, or add it as a Teams tab.
Responses are automatically collected in a spreadsheet (click 'Open in Excel' to analyze results) or viewable in the built-in Forms response dashboard with charts. Set a start and end date to control when the form accepts responses. For quizzes, add correct answers and point values — Forms calculates scores automatically and can show respondents their results immediately. Export results to Excel for further analysis or reporting.
